Job Title: Band 5 Community Nurse

Hours: Flexible shifts, available 7 days per week  

We are currently seeking dedicated and compassionate Band 5 Community Nurses to join our team. If you are passionate about providing care in the community and enjoy working independently, this is an excellent opportunity for you!

Key Responsibilities:
- Deliver high-quality nursing care to patients in the community, assessing, planning, and evaluating their individual needs.
- Work independently to manage a caseload of patients, ensuring all nursing interventions are carried out efficiently and effectively.
- Maintain accurate and timely patient records and adhere to all clinical governance standards.
- Provide patient education and support to promote self-management where appropriate.
- Collaborate with multi-disciplinary teams to ensure coordinated care for patients.
  

Requirements:
- A valid NMC Pin is essential.
- Minimum of 6 months of NHS community nursing experience
- A full, clean UK driving license and access to a vehicle for community visits.
- Excellent communication and interpersonal skills.
- Ability to work autonomously and manage a caseload effectively.
- Flexible to work shifts across 7 days per week, including weekends and bank holidays.
