Position: Practice Nurse
Hours: Full-time/Part-time available

About the Role:

The successful candidate will play a crucial role in delivering high-quality patient care within a supportive and collaborative environment. As a Practice Nurse, you will be responsible for managing a varied caseload, including chronic disease management, immunizations, wound care, and health promotion. You will work closely with GPs, healthcare assistants, and administrative staff to ensure the smooth running of the practice and the best outcomes for patients.

Key Responsibilities:

• Conducting health assessments and providing evidence-based care to patients.
• Managing long-term conditions such as diabetes, asthma, and hypertension.
• Administering vaccinations, including travel immunizations and flu jabs.
• Providing wound care and assisting with minor surgical procedures.
• Offering advice and support on family planning, smoking cessation, and other health promotion activities.
• Participating in quality improvement initiatives and audits.
• Maintaining accurate patient records in line with best practice guidelines.

Qualifications and Experience:

• Registered Nurse with a valid NMC PIN.
• Previous experience in a primary care or community setting is desirable.
• Proficiency in chronic disease management and health promotion.
• Strong communication and interpersonal skills.
• Ability to work autonomously and as part of a multidisciplinary team.
• Commitment to continuous professional development.
